How to Create a Quotify Appointment Form

 

        Login to your dashboard.

        Go to Setup > Custom Field > New Custom Field.

        Make sure Belongs To is set to Leads.

        Enter the Field Name (e.g., Appointment Date).

        Choose the appropriate Field Type (e.g., Date & Time Picker).

        Click Save.

        Go to Setup > Leads > Web to Lead > New Form.

        Enter a Form Name and Source (e.g., Website Appointment).

        Set a Default Status (e.g., Pending).

        Assign this form to a staff member for tracking and notifications.

        Click the Branding tab to customize the button color, style, and text.

        In the Submission tab, enable Allow Duplicate Leads, set a Thank You Message, or redirect to your landing page.

        In the Notification tab, assign staff and enable alerts.

        Click Save to create the form.

        The system redirects to the Form Builder, where you can drag and drop fields like Name, Email, Phone, Appointment Date, etc.

        After designing the form, click Save.

        Go to Integration Code to copy the embed code or sharable link for your website or Quotify widget.
